Terms and Conditions:

1. This installment plan is perpetual until further notice.  
2. This installment plan includes surgical, non-surgical procedures, ZO kits combined with other products  and/or services. Packaged treatments are also included except for Lumina Whitening System, Acne Basics,  and Lumina Basics. 
3. This installment plan is applicable to the Belo Shop, for BDO and Metrobank cardholders only.  
4. Patients may choose which installment plan they want to avail.  
5. ZO card holders/members are NOT included in the 0% interest program.  
6. This can’t be used in conjunction with other discounts and promos unless approved by the Managing  Physician or unless mentioned on a different memorandum. 
7. Those services under standard exclusions are only eligible for 3 months or 6 months.
